Support Coordinator (NDIS)

Job Summary:

We are looking for an organised and compassionate Support Coordinator to help NDIS participants get the most out of their plans. As a Support Coordinator, you will work closely with clients to understand their NDIS funding, connect them with service providers, and coordinate supports that align with their personal goals. Your role is pivotal in empowering individuals to navigate the NDIS system and build their capacity for independence. Responsibilities:

• Assist NDIS participants in understanding their plans and funding, explaining available support options clearly.
• Coordinate and connect participants with suitable service providers (therapy, care services, community activities, etc.) based on their needs and goals.
• Monitor the progress and effectiveness of supports, helping clients adjust services as needed and manage their plan budgets.
• Advocate for participants’ rights and choices, ensuring their voice is heard in the planning and service delivery process.
• Prepare documentation and reports for NDIS reviews, and maintain regular communication with clients, families, and other stakeholders.

Skills and Qualifications:

• Experience in case management, support coordination, or social work within the disability or community services sector.
• Thorough understanding of the NDIS framework, funding categories, and local support services.
• Strong organisational and multitasking abilities to manage multiple client plans efficiently.
• Excellent interpersonal skills with empathy, cultural sensitivity, and the ability to empower others.
• Relevant qualifications such as a Diploma or Degree in Social Work, Disability, or Community Services; plus a current driver’s licence and NDIS Worker Screening clearance.
